2. Fill out the form completely (sample shown below)

3. Bring the form to the Schiffert Health Center Pharmacy (in Room 117 McComas Hall during our hours of operation) and receive your requested over-the-counter medications.

CAN'T PRINT THE LINK?

Pick up a copy of the form (sample shown below) from the lobby of Schiffert Health Center in the Self-Care Cold Clinic. (hours of operation). Completely fill it out and take it to the SHC Pharmacy in Room 117 McComas Hall to receive your requested medications. NO APPOINTMENT NECESSARY! No money needed!

REMEMBER: USE COLD SELF-CARE TECHNIQUES TO HELP YOU MANAGE YOUR COLD SYMPTOMS. SEEK AN APPOINTMENT WITH A HEALTHCARE PROVIDER IF YOU DO NOT THINK YOUR SYMPTOMS ARE GRADUALLY IMPROVING WITHIN 7 DAYS AND/OR YOU EXPERIENCE OTHER SYMPTOMS.
