Immunization History
Requirements
The University does not require a complete physical examination, except for members of the Corps of Cadets. All full-time students at Virginia Tech must meet immunization requirements as set by Commonwealth of Virginia Law. If you have accepted an offer of admission to Virginia Tech please download the form and instructions (five pages) on the right. The completed form is to be mailed to:

On-Line Student Health Portal (OSH)
Virginia Tech students may create an account on the Schiffert Health Center On-Line Student Health Portal (OSH). In OSH, students may view their immunization status and missing university immunization requirements.
Students may also enter their immunization
history in the OSH; however a signed and
completed immunization form must
still be mailed to Schiffert Health Center.
Students are not required to use the OSH.
How to use the On-Line Student Health
portal:
1) Navigate to
https://www.healthcenter.vt.edu/OSH
2) Register to create a OSH account - you will need your
Virginia Tech ID number and Virginia Tech e-mail
address.
3) A password will be sent to your Virginia Tech
email address and you will be asked to change
the password. Passwords must be at
least 8 characters long and contain at least one
upper case letter, lower case letter, and
number. Example: "xTp9z32m".
4) Remember your user name and password - don't
share it. If you forget your user name or
password, you will need to register with OSH
again.
