Advance Directives

An advance directive is a written statement of a person’s wishes regarding medical treatment that is created to ensure that those wishes are carried out in the event that the person is unable to communicate them to a doctor. Below are two resources for Virginia residents. The first lets you know about your rights under Virginia law about making decisions about your health care, and the second is the Virginia Advance Directive for Health Care form, which you can fill out and submit to any Virginia health care institution (including Schiffert Health Center).
