Medical Records

Your medical record is kept confidential at all times and is not associated with your academic record.  Medical records are retained for a period of ten (10) years following graduation or last treatment at VT SHC.

You may obtain a copy of your medical record if you wish.  If you request a copy of your entire medical record, or a large portion of it, there is a $10.00 charge.  This is due and payable upon request of the copy.  To obtain a copy of your medical record, you need to:

1. Print, complete, and sign a copy of the AUTHORIZATION FOR RELEASE OF INFORMATION form (requires Acrobat Reader)
2. Make out a check for $10.00 made payable to "University Student Health Services."
3. Mail the form and check (or deliver) to: Virginia Tech, Schiffert Health Center, McComas Hall, Blacksburg, VA 24061-0140, ATTENTION: Medical Records.
