Your Rights as a Patient at VT SHC
- to be treated with respect and consideration.
- to obtain complete and current information concerning your diagnosis treatment and prognosis.
- to be seen by the practitioner of your choice.
- to refuse treatment and to be informed of the medical consequences of this action.
- to expect that communications and records are treated confidentially as required by law.
- to understand why procedures and tests are required.
- to examine and receive an explanation of your SHC bill regardless of source of payment.
- to know what SHC rules and regulations apply to your conduct as a patient.
- to refuse to participate in any research projects.
Your Responsibilities as a Patient at VT SHC
- to meet all eligibility requirements.
- to arrive on time for your appointments and follow-up visits, and to phone SHC if you must cancel or if you will be late.
- to provide the SHC with a complete medical history form submitted before the stated deadline and noting :immunizations, past illnesses, hospitalizations, medications, and other health matters.
- to ask questions if any aspect of your treatment is not clear to you.
- to follow directions concerning medication, treatment, and rest/activity.
- to treat SHC staff with respect and consideration.
