Patient Rights and Responsibilities

Your Rights as a Patient at Schiffert Health Center:

  • To be treated with equality and respect 
  • To affirmation of your true gender identity
  • To obtain complete, current information concerning your diagnosis, treatment, and prognosis
  • To be seen by the healthcare provider of your choice
  • To refuse treatment and to be informed of the medical consequences of this action
  • To expect that communications and records are treated confidentially as required by law
  • To understand why procedures and tests are required
  • To examine and receive an explanation of your medical bill regardless of source of payment
  • To refuse to participate in any research projects
  • To ask for a second opinion regarding diagnosis or treatment by either another practitioner within Schiffert Health Center, or an outside provider. Please note that consultation outside Schiffert Health Center is your financial responsibility
  • To change your health center care provider. Please inform the receptionist/triage nurse when you make an appointment
  • To know that your Schiffert Health Center medical record will be destroyed 6 years after graduation or your last visit to SHC
  • To obtain Schiffert Health Center’s privacy statement upon request

Your Responsibilities as a Patient at Schiffert Health Center:

  • To meet all eligibility requirements
  • To arrive 10 minutes before an appointment and to phone/email Schiffert Health Center if you must cancel
  • To know what Schiffert Health Center rules and regulations apply to your conduct as a patient
  • To provide Schiffert Health Center with an immunization history form that is submitted before the deadline and notes immunizations
  • To ask questions if any aspect of your treatment is not clear to you
  • To follow directions concerning medication, treatment, and rest/activity
  • To treat staff with the same respect and consideration that you would expect from Schiffert Health Center staff

Schiffert Health Center is accredited by the Accreditation Association for Ambulatory Health Care.
